Student Statement of Financial Responsibility
This Statement is available in your MyFresnoState portal. In the portal, you must read and agree to the following Student Statement of Financial Responsibility in its entirety.
Student Statement of Financial Responsibility
1. I agree that by registering for a course or courses, attending or not, I am incurring
a financial liability, and I understand the balance due
must be paid by the established payment deadline. Failure to pay the balance due may subject me to denial of services and/or being
sent to
collections and credit reporting.
2. I understand it is my responsibility to keep my mailing address, home address and
other general information current on my student account. I
acknowledge that failure to do so may result in not receiving critical information
such as bills, late notices, Franchise Tax Board intercept notices,
and collection notices regarding collection actions that may have a negative impact
on my credit.
3. I authorize Fresno State and its agents and contractors to contact me at my current
and any future cellular phone number(s), home telephone
number(s), email address(es) or wireless device(s) regarding my delinquent student
account(s)/loan(s), and any other debt owed to Fresno State,
or receive general information from Fresno State. I authorize Fresno State and its
agents and contractors to use automated telephone dialing
equipment, artificial or prerecorded voice or text messages, and personal calls and
emails in their efforts to contact me. Furthermore, I
understand that I may withdraw my consent to call my cellular phone by submitting
a request in writing to the Student Accounts section of the
Accounting Services Department or in writing to the applicable contractor or agent
contacting me on behalf of Fresno State.
4. I understand Fresno State will periodically send courtesy emails to my official
University Fresno State email account, informing me that my
electronic bill is available. Not viewing or receiving the electronic bill does not
relieve me of the responsibility to pay the balance due.
5. I understand I am consenting to receive my annual IRS Form 1098-T and billing statement, electronically
via MyFresnoState. I understand
that I can withdraw this consent and receive a paper copy by emailing Student Accounts
at student_financial_services@listserv.csufresno.edu
before the distribution of the 1098-T with the subject line “1098-T opt out”. Emails
must be sent from your Fresno State email account and your
mailing address must be up to date.
6. I understand it is my responsibility to provide a Social Security Number or Individual
Taxpayer Identification Number to the University for
1098-T reporting. Failure to comply may result in a $50 penalty, assessed by the IRS
under Section 6723.
7. I understand payments made online or mailed, that are not received in Cashiering
Services by December 15, 2025, may not be posted to my
2025 1098-T due to the December campus closure.
8. I understand any financial aid, scholarships, fee waivers, sponsorships, or other
aid awarded to me may be deducted from the balance owed
to the University; however, if any of these items are reversed, reduced or less than
expected for any reason, then I understand it is my
responsibility to pay any remaining balance due by the established payment deadline.
9. I understand I may be dis-enrolled from all courses if:
- I fail to pay the balance due by the payment deadline; or
- I do not have a fee deferment on my student account.
10. I understand that I may be disenrolled from all classes if I fail to pay all fees
by the deadline and I may owe full or prorated fees for all classes
that are dropped on or after the first day of instruction due to disenrollment.
11. I understand I am responsible for dropping my classes. Failure to do so may result
in full or prorated fees owed and forfeiture of all tuition
and other fees paid to date.
12. I agree to read and adhere to the University’s Refund Policy and Deadline Dates. If I drop courses on or after the first day of instruction*, I
may owe full or prorated fees.
13. Cancellation/Adjustment of Registration Fees
A. For state-supported semesters, and non-standard terms or courses of four (4) weeks
or more:
(a) A student who drops all courses prior to the first day of instruction* for that term shall be entitled to an adjustment of all tuition and
mandatory fees paid.
(b) A student who officially drops courses within the first 10 days of instruction*,
shall be entitled to an adjustment of tuition
and mandatory fees less an administrative fee of $5.
14. Withdrawals
A. For state-supported semesters, and non-standard terms or courses of four (4) weeks
or more:
(a) A student who withdraws from all courses in accordance with university policy
and procedures on or after the first day of instruction*
and up to a date that includes the sixty percent point in the academic period, is
eligible for a prorated reversal of tuition and mandatory
fees based on the proportion represented by the number of days in the academic period,
excluding breaks of five (5) days or more.
(b) A student who officially withdraws from the University after the sixty percent
point in the academic period or fails to drop courses prior
to the end of the University drop period, or fails to officially withdraw from the
University, is not eligible for a reversal of tuition and
mandatory fees
B. For state-supported semesters, and non-standard terms or courses of less than four
(4) weeks:
(a) Students registered for any course, state-supported or academic period, that is
less than four (4) weeks in duration are not eligible to
any reversal of tuition and mandatory fees, if the student fails to either drop the courses or cancel registration prior to the first day of
instruction.
15. I understand it is my responsibility to read and adhere to the Continuing and Global Education policies regarding payment, add/drop dates,
procedures, and deadlines - Registration Information.
16. The CSU makes every effort to keep student costs to a minimum. Fees listed in
published schedules or student accounts may need to be
increased when public funding is inadequate. Therefore, CSU must reserve the right,
even after fees are initially charged or initial fee payments
are made, to increase or modify any listed fees. All listed fees, other than mandatory
systemwide fees, are subject to change without notice,
until the date when instruction for a particular semester or quarter has begun. All
CSU listed fees should be regarded as estimates that are
subject to change upon approval by the Board of Trustees, the Chancellor, or the Presidents,
as appropriate. Changes in mandatory systemwide
fees will be made in accordance with the requirements of the Working Families Student
Fee Transparency and Accountability Act (Sections
66028 - 66028.6 of the Education Code).
* First day of instruction is defined by the Academic Calendar, which is submitted
by the Provost and approved by the Chancellor’s Office.
