etc.) for which you did not receive or use the services that were originally charged.

Used in requesting reversal of prorated tuition fees as a result of withdrawing from
classes after the first day of instruction. Petition requests require supporting documentation of a significant change in circumstances which prevented you from dropping classes
before the first day of instruction and/or after the drop deadline.
Submission of the Tuition & Fees petition request form does not guarantee release from your financial responsibility or reversal of tuition fees will be approved.
